This position requires you to be on-site in Helena. This position is responsible for researching, developing, and implementing a complex care position with a focus on Alzheimer's disease and related dementias (ADRD). The position will serve as a lead case manager and will report directly to the Executive Director, Medicaid and Health Services Branch. The position will be responsible for designing, developing, and implementing a complex care management initiative, with support from Department leadership. The member population will include significantly complex medical conditions and/or mental health co-morbidities. This position will be responsible for managing multiple highly complex cases simultaneously.
