About The Position

The Community Wellness & Nutrition Coordinator plays a vital role in SMAA’s integrated approach to healthy aging to deliver cohesive, strategic, community-based nutrition and wellness programs. The Coordinator oversees congregate and home-delivered meal program (Meals on Wheels) activities at our South Portland area meal sites, supports evidence-based health promotion classes, recruits and manages volunteers, and builds partnerships to expand access to programs in high-need and underserved areas. This role directly supports SMAA’s strategic priorities in Social Care Coordination, Volunteer Innovation, and Community Engagement across its centers in Cumberland and York Counties.

Requirements

  • Associate’s degree or higher in social work, public health, nutrition, or related field, or equivalent experience.
  • At least two years of experience in community or nutrition program coordination, preferably with older adults.
  • Experience recruiting and managing volunteers.
  • Exceptional organizational, interpersonal, and communication skills.
  • Strong ability to multitask, prioritize, and work independently.
  • Comfort with data systems (e.g., WellSky, Trumba, Better Impact, Excel).
  • Demonstrated cultural competence and commitment to inclusion and equity.
  • Ability to lift and carry up to 50 pounds (meals and program materials).
  • Valid driver’s license, reliable transportation, and acceptable driving record required.

  • Experience facilitating or supporting community health or wellness programs preferred.

Responsibilities

  • Coordinate the planning and implementation of SMAA’s Agewell and Nutrition site activities, including congregate dining, home-delivered meals, and evidence-based health promotion classes.
  • Support HDM and Congregate site operations.
  • Serve as on-site contact for day-to-day operations, ensuring evidence-based program fidelity, participant satisfaction, and adherence to safety and sanitation standards.
  • Collaborate with SMAA’s Volunteer Services Director and Community Outreach Team to plan wellness classes aligned with organizational goals and agency-wide partnership efforts: at SMAA hubs and in partnership with Age-Friendly (AF) communities, communities high on the Social-Vulnerability Index (SVI), and Ethnic and Community-Based Organizations (ECBOs).
  • Manage all program supplies.
  • Collaborate with the Volunteer Services team to recruit, onboard, and train volunteers for meal and wellness programs.
  • Foster volunteer socialization and connection.
  • Coordinate wellness volunteer trainings and certifications in EBPs; track and ensure volunteers maintain their EBP certifications and ongoing EBP training requirements.
  • Lead and organize regular volunteer team meetings, recognition events, and milestone celebrations.
  • Provide ongoing mentorship and support to volunteers.
  • Conduct client assessments to identify nutrition, wellness, or social needs, referring to appropriate SMAA programs as necessary.
  • Support volunteers with accessing additional supports for clients who experience needs outside of the meals and Agewell programs; work with staff of SMAA and other appropriate providers to connect clients to appropriate benefit programs and services that they need and for which they qualify.
  • Maintain a welcoming, inclusive environment that supports client dignity and engagement.
  • Respond to nutrition client and wellness class participant inquiries via phone, email, or in-person visits promptly and courteously.
  • Collaborate with SMAA’s Director of Community Outreach to develop and sustain partnerships with local organizations to enhance outreach and expand service delivery, particularly in high-Social Vulnerability Index (SVI) communities, with AF communities, and with ECBOs.
  • Collaborate with SMAA’s Outreach and Advancement teams on marketing, storytelling, and media opportunities.
  • Represent SMAA at community events, classes, and outreach fairs to promote programs and recruit volunteers and participants.
  • Manage participant and volunteer records across multiple systems including WellSky, Trumba, and Better Impact.
  • For wellness classes, share final registration lists with class leaders; provide volunteer class leaders with updates on individuals and numbers registered as needed prior to class start date.
  • Administer and analyze post-program surveys and volunteer evaluations; analyze and share results to inform continuous improvement.
  • Prepare periodic reports and maintain accurate documentation in compliance with SMAA and program standards.
  • Complete training for Master Trainer’s certification for A Matter of Balance within one year; begin work toward certification as a Tai Chi instructor within one year. Become certified in any other EBP the agency may decide to offer.
  • Serve as the agency's resident expert in EBP program fidelity.
  • Participate in SMAA professional development and interdisciplinary team meetings.
  • Maintain confidentiality of all matters whether it be of client-related issues or Agency business.
  • Ensure that work area and work practices focus on safety and reporting all work-related incidents via an incident report immediately and any possible work-related safety concerns to the Supervisor or designee.
  • Perform other duties as assigned.
