About The Position

Under the direction and guidance of the Supported Living Facilitator, the Community Support Staff-Floater (CSSF) is responsible for providing home-based support and assistance to individuals with developmental disabilities served by Las Trampas Supported Living Services. The primary purpose of the CSS-Floater is to fill in shifts at various homes that are open including those that are open due to illnesses, vacations, leaves of absence, unfilled regular positions, etc. An open, flexible schedule is required for this position and shifts to be filled will include all available shifts (daytime hours, overnights, weekends, etc.). The CSSF is responsible for understanding the goals and objectives of the individuals served as outlined in the individuals’ Individual Program Plans (IPPs) and Individual Service Plans (ISPs). The CSSF assists individuals served in meeting their personal needs, supports needed, and training in the acquisition of skills needed for individuals served to maintain community placement in a home of their own, respecting the personal rights of individuals with developmental disabilities, and providing for the care, welfare and safety of the individuals served. The CSSF is also responsible for providing services and maintaining the Five Principles of Supported Living and consumers’ full participation in their lives and the responsibilities of an adult life. The CSSF is responsible for adhering to California Code of Regulations, Title 17, Consumer Rights, and all aspects of the Individual Service Plan (ISP) process.

Requirements

  • High School diploma or equivalent.
  • Must be 18 years of age or older.
  • Must be willing to work for any individual served in the Supported Living Program regardless of location.
  • Have an open, flexible schedule.
  • Good written and verbal communication skills.
  • Adequate knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Fundamental math and budgetary skills and ability to assist individuals with maintaining a household budget.
  • Valid California driver’s license, acceptable DMV printout, and proof of vehicle insurance.
  • Reliable vehicle for transportation of individuals with developmental and/or intellectual disabilities.
  • Maintain current and valid First Aid and CPR training.
  • Pass a criminal background check through the Department of Justice.
  • Ability to lift 50 pounds and assist individuals with transfers to and from mobility equipment and vehicles.
  • Pass a pre-employment physical indicating ability to perform the essential functions of the position as stated below.

Nice To Haves

  • Six months’ experience working with persons with developmental and/or intellectual disabilities or in a human services field preferred.

Responsibilities

  • Be flexible with scheduling and fill open shifts as they come available.
  • Be flexible in working in various Home environments and under the direction of various Supported Living Facilitators.
  • Provide direct service, support, and personal assistance services for individuals served by the program (personal hygiene and grooming needs, shopping, administering medications, laundry, general household cleaning, community access, medical and/or dental appointments, etc.).
  • Assist individuals in increasing and/or maintaining their quality of life and attaining their goals and objectives as outlined in the IPP and ISP.
  • Assist individuals in developing their Circle of Support within the community by building and maintaining relationships with family members, friends, neighbors, referral source agency, community resource agencies, and others the individual feels are important to the maintenance of their independence.
  • Encourage active participation from members of the individual’s Circle of Support.
  • Ensure that all generic resources such as outside agencies and members of the Circle of Support are primary support agents (e.g., In-Home Support Services, subsidized housing, Social Security, Supplemental Security Income, etc.).
  • Provide transportation for the individuals as necessary to scheduled and unscheduled appointments and activities necessary for maintaining independence in the community.
  • Monitor the health and medical needs of the individuals and support the individuals in all aspects of maintaining adequate medical care.
  • Support individuals to effectively manage their finances.
  • Assist with plans for the reduction of expenses while observing personal rights to choose.
  • Attend all mandatory and required in-service training, staff meetings, conferences and workshops.
  • Respect and support individual choices, routines, and preferences and abide by the principles of person-centered planning.
  • Responsible for logging all trips in agency-owned and personal vehicles for reimbursement consideration.
  • Assist individuals in the acquisition and use of adaptive equipment if necessary or needed.
  • Provide 24-hour emergency response when threats to the individual’s health, safety, and/or welfare are at risk and provide continuous and consistent support for individuals, thereby ensuring their safety in the community.
  • Provide training to individuals and members of the Circle of Support in emergency disaster planning.
  • Proactively communicate with supervisors regarding the individuals’ needs and threats to health, safety, and welfare.
  • Proactively communicate with supervisors regarding the progress and barriers to success for the individuals served.
  • Respect confidential and proprietary agency information by not sharing with such information with other organizations.
  • Avoid any conflict of interest.
  • Maintain the confidentiality of the individuals served.
  • Ensure that all documentation on consumer progress, communications between staff members and outside agencies is completed thoroughly, accurately, and timely.
