About The Position

The Community Support Specialist provides community support and case coordination services for a caseload of individuals with disabilities/intellectual disabilities or individuals with mental illness. The Community Support Specialists works with the people we serve using a true person center approached in order to foster relationships between individuals and their communities. Requirements

  • Bachelor's degree from four-year college or university in the human services field
  • At least one year of direct care experience working with persons with disabilities.
  • Must be able to achieve Qualified Intellectual Disability Professional (QIDP) status to work with persons with developmental disabilities.
  • Must have Mental Health Professional (MHP) status to work with persons with mental illness.
  • Must have reliable transportation, valid driver's license, proof of insurance, and meet company insurability standards by having a good driving record.
  • Employment with Bridgeway is conditional upon completion of all background checks.

Responsibilities

  • Assess consumer's overall functioning, identifying problems, needs, skill strengths and deficits, and assume a lead role in the consumer centered planning process developing implementation strategies.
  • Implement selected aspects of the plan including skill building, counseling, advocacy, and building community, family, and natural supports.
  • Ensure linkages to community service providers, entitlement programs and housing alternatives.
  • Advocate for individual's basic need, development of natural resources, and personal independence through teaching self-advocacy skills.
