Requirements

  • Must be at least twenty-one (21) years of age to operate a company-owned vehicle
  • Must be at least twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van
  • Valid driver’s license with acceptable driving record and current auto insurance
  • Reliable transportation required for transporting clients in personal vehicle
  • Successful completion of background checks including criminal, driving, abuse/neglect, and fingerprint checks
  • Ability to obtain required trainings and certifications to meet DMH standards
  • Strong communication, problem-solving, and organizational skills
  • Ability to build effective relationships with adults and their families while maintaining professional boundaries
  • Knowledge of community resources, case management principles, and confidentiality laws related to behavioral health

Responsibilities

  • Provide community-based mental health services to adults through assessment, treatment planning, and intervention implementation
  • Collaborate with clients and families to develop individualized, recovery-oriented treatment plans
  • Utilize strengths-based and evidence-based practices including Motivational Interviewing, Trauma-Informed Care, CBT, and DBT-informed strategies
  • Deliver behavioral interventions that promote skill-building, emotional regulation, and resilience
  • Coordinate care with community partners such as primary care providers, psychiatrists, and local organizations
  • Prepare for and facilitate PSR group sessions aligned with treatment plan goals
  • Maintain accurate and timely documentation of services, progress, and outcomes in accordance with organizational standards
  • Participate as an active member of the interdisciplinary treatment team to ensure coordinated care
  • Build and maintain professional relationships with clients, families, and key community stakeholders
  • Contribute to quality improvement initiatives through data collection, reporting, and participation in team efforts
  • Attend required trainings, meetings, and professional development activities
  • Perform additional duties as assigned by leadership
