About The Position

The Community Support Professional provides direct support and guidance to individuals with developmental disabilities, promoting independence, inclusion, and personal growth in alignment with the principles of Everyday Lives. This role facilitates meaningful participation in community life, supports individual goals, and ensures health, safety, and dignity across all environments.

Requirements

  • Knowledge of community resources and activities across social, cultural, educational, and recreational domains.
  • Ability to support individuals with diverse medical, emotional, and behavioral needs.
  • Strong interpersonal skills and professional demeanor.
  • Capacity to manage challenging behaviors and emergency situations.
  • Commitment to inclusion, dignity, and respect for all individuals.
  • High School Diploma or equivalent required.
  • Valid driver’s license and eligibility for insurance coverage.
  • CPR and First Aid certification (must be maintained).
  • Medication Administration certification (as designated).
  • Acts 33 & 34 clearances required.
  • FBI clearance required, if applicable.
  • Physical exam and tuberculosis test required and maintained per regulations.

Nice To Haves

  • One year of direct care experience with individuals with developmental disabilities preferred.
  • For Community Participation Support enhanced services, must possess and maintain the relevant certification or degree per the service definitions.

Responsibilities

  • Understand and implement each assigned individual’s support needs as outlined in the Individual Support Plan (ISP).
  • Provide supervision to the individual in accordance with their identified staffing ratio in the ISP.
  • Promote autonomy, choice, and independence in daily activities, relationships, and community participation.
  • Support individuals in vocational, recreational, educational, cultural, and spiritual activities that foster social capital.
  • Assist with personal care needs including feeding, toileting, transferring, and hygiene, as required.
  • Have knowledge of individual diets and monitor during mealtimes.
  • Facilitate access to community resources and activities that align with individual interests and goals.
  • Encourage career exploration and employment-first initiatives.
  • Plan and implement community outings, volunteer opportunities, and other integrated programs.
  • Ensure the health and safety of individuals in all settings.
  • Complete Change of Health form when appropriate.
  • Obey all traffic laws while driving for work purposes.
  • Immediately report vehicle accidents to supervisor after the health and safety of individuals has been ensured.
  • If certified for medication administration, follow the medication administration training guidelines and report medication errors immediately.
  • Respect and uphold individual rights.
  • Respond to and report suspected abuse or incidents per the Incident Management Policy.
  • Implement emergency procedures and support individuals during evacuations.
  • Use communication methods appropriate for each individual, following the Communication Plan if applicable.
  • Understand and implement behavior plans and treatment recommendations.
  • Complete Record of Behavioral Event when appropriate.
  • Actively listen and respond respectfully to individuals’ ideas and concerns.
  • Participate in team meetings and contribute to ISP development.
  • Communicate relevant information to team members and supervisors.
  • Maintain accurate and timely documentation, including health or communication logs, day service notes, client employee production or work time, and reports.
  • Attain proficiency in electronic recordkeeping systems.
  • Complete assessments and contribute to goal planning and progress tracking.
  • Assist in training new staff and guide potential employees during site visits.
  • Maintain a clean and organized workspace and request necessary supplies or repairs.
  • Represent the organization professionally to families, community members, and stakeholders.
