About The Position

Residential Transitions Inc. (RTI) is seeking a dedicated Community Services Program Manager . The primary purpose of the Community Services Program Manager is to oversee waiver programs and manage employees who provide Individualized Home Supports (IHS with training) & 24-hour Emergency Services to participants in the community. This includes clinical evaluations of services at a programmatic level and with individual participants with a diagnosis of mental illness, developmental disability, and/or brain injury.

Requirements

  • The Program Manager shall be qualified under Minnesota law as follows :
  • Bachelor’s degree in a field related to human services; and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
  • OR Associate degree in a field related to human services; and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
  • OR Diploma from an accredited postsecondary institution in a field related to human services; and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
  • OR A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, under the supervision of a staff person who meets one of the above qualification combinations (Bachelor’s, Associate, or Diploma with required experience)
  • At least 2 years of experience working in community based services.
  • Willingness to travel within the Metro Area, for applicable mileage reimbursements, with a current driver’s license, clean record, and personal vehicle with insurance; and able to transport clients in that vehicle.
  • Must pass the required background check and remain eligible to provide services as defined by the Minnesota Department of Human Services.

Responsibilities

  • Staff Management and Supervision
  • Supervise Mental Health Workers who provide IHS support and 24-hour Emergency Services.
  • Provide guidance, direction, and performance feedback to employees.
  • Conduct annual performance reviews and ensure staff meet training requirements.
  • Make hiring and termination recommendations for direct care staff.
  • Participant and Clinical Support
  • Offer clinical and professional support to enhance participant development in coping skills, employment readiness, and independent living.
  • Meet with potential participants and providers to share information about RTI’s services.
  • Complete intake assessments and documentation such as IAPP, CSSPA, Self-Management Assessments, and goal planning.
  • Program Oversight and Development
  • Support development and implementation of new programs and services.
  • Ensure person-centered approaches are reflected in service delivery.
  • Monitor and ensure licensing compliance for all assigned programs.
  • Emergency Services and Crisis Management
  • Manage 24-hour Emergency Support Services including shift assignments and on-call scheduling.
  • Provide direction during mental health crises and guide staff on handling verbal or physical aggression.
  • Apply de-escalation techniques and motivational interviewing to support crisis stabilization.
  • Team Coordination and Collaboration
  • Facilitate interdisciplinary team meetings and collaborative planning with other programs or external agencies.
  • Lead communication across internal departments and external case managers to support participant success.
  • Administrative Duties and Compliance
  • Review participant progress notes and ensure service fidelity.
  • Maintain documentation of significant events related to staff and participants.
  • Complete incident reports and follow-up actions as required.
  • Develop and manage program schedules and budgets.
  • Undertake additional projects as assigned.
