About The Position

The position is responsible for promoting the efficient and effective use of a behavioral health care by focusing on the whole patient to provide a seamless continuum of care for Oyate Health Center (OHC) patients. This position consults with patients and/or their families to facilitate achieving treatment plan goals, developing life skills, and/or sustaining financial stability, conducts activities and monitors the service environment in a manner to maximize patient success and well-being.

  • Bachelor s Degree and one (1) year of experience, or associate s degree and three (3) years of experience, or high school diploma or GED and five (5) years of progressively responsible experience. Progressively responsible work experience may be substituted on a year-for-year basis for college.
  • Must successfully pass a criminal and background check, and a pre-employment drug screen.

You are required to agree that you will comply with all job-related employee health screening and immunizations prior to your first day of employment. Jobs require that you have documentation that you have completed the following immunizations prior to your first day of employment: MMR (Measles, Mumps and Rubella, Varicella (Chicken Pox), Hepatitis B, Influenza, T-dap (Tetanus - Diphtheria - Pertussis), and COVID-19 vaccination is required .

Responsibilities

  • Facilitate patient access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services; assist to develop resources and make contact with social support networks.
  • Assist patients in developing goals and areas of need, assists in developing treatment plans which are assessed regularly.
  • Work with agencies and service providers to facilitate case coordination and information sharing.
  • Coordinate and monitor services, including comprehensive tracking of patient activities in relation to care plan.
  • Document all patient encounters; complete and submit billing documentation as appropriate; maintain comprehensive patient files.
  • Identify and provide emergency crisis services as necessary; make immediate assessment and respond according to accepted crisis intervention methods and techniques; coordinate other services as appropriate.
  • Assess patients, evaluates effectiveness of care plan and progress made by client, participate in patient treatment planning and case review with patient care providers.
  • Promote effective and efficient utilization of clinical resources.
  • Develop effective working relations and cooperate with medical team throughout the entire case management process.
  • Maintains and reports applicable statistics regarding programs and patient services.
  • Must comply with federal laws and regulations as required by the Health Insurance Portability and Accountability Act (HIPAA).
  • Performs miscellaneous job-related duties as assigned.
  • Other duties as assigned by the Supervisor
