As a Community Resource Specialist (CRS) at One Health , you’ll be a frontline advocate for patients, connecting them with vital resources and support systems. This patient-facing role is essential in helping our most vulnerable community members overcome barriers to care and improve their overall wellbeing. Requirements
- Strong computer and documentation skills
- Ability to manage multiple priorities with remote supervision
- Professionalism, empathy, and clear boundaries
- Community involvement or connection to the population served
- High School Diploma or GED
- Valid Driver’s License
- Certifications (upon hire or within first year): BLS Certificate Community Health Worker Certificate Certified Application Counselor (CAC)
Responsibilities
- Coordinate Care: Facilitate warm hand-offs and referrals between patients, providers, and community services.
- Screen & Support: Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers.
- Educate & Advocate: Assist with applications for coverage, benefits, and other essential services.
- Document Thoroughly: Maintain detailed electronic patient records, tracking outcomes and service plans.
- Engage the Community: Participate in outreach events and consortiums to promote One Health services.
- Navigate Resources: Stay current on eligibility rules, insurance options, and enrollment procedures.
