This patient-facing role is essential in helping our most vulnerable community members overcome barriers to care and improve their overall wellbeing. Requirements

  • Strong computer and documentation skills
  • Ability to manage multiple priorities with remote supervision
  • Professionalism, empathy, and clear boundaries
  • Community involvement or connection to the population served
  • High School Diploma or GED
  • Valid Driver’s License
  • Certifications (upon hire or within first year): BLS Certificate Community Health Worker Certificate Certified Application Counselor (CAC)

Responsibilities

  • Coordinate Care: Facilitate warm hand-offs and referrals between patients, providers, and community services.
  • Screen & Support: Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers.
  • Educate & Advocate: Assist with applications for coverage, benefits, and other essential services.
  • Document Thoroughly: Maintain detailed electronic patient records, tracking outcomes and service plans.
  • Engage the Community: Participate in outreach events and consortiums to promote One Health services.
  • Navigate Resources: Stay current on eligibility rules, insurance options, and enrollment procedures.
