The Community Resource Specialist focuses on connecting the social determinant of health (SDOH) needs of the patient with available community resources.

Requirements

  • High school diploma or general education degree (GED)
  • Achieves Community Health Worker certification requirements within 6 months of employment and maintains certification while employed.
  • Excellent interpersonal communication and problem-solving skills
  • Skills to intervene and promote reconciliation, compromise and positive outcomes in difficult interactions
  • Bilingual in English/Spanish

Nice To Haves

  • Associates degree in related field
  • Basic knowledge of adult learning models
  • Skilled at using electronic health records as applicable to area of work (EPIC)

Responsibilities

  • Screens/assesses for social determinants of health (SDOH) needs and connects patients to resources within the community.
  • Provides relevant community and/or health care resources, assisting patients in navigation of accessing those resources when indicated.
  • Ensures the documentation of patient care in EPIC and is responsible for following workflows that result in capturing activities to meet patient engagement standards as outlined in the APCM program.
  • -CareSTEP: Online Portal Engagement
  • -CareSTEP: Accessing Community Resource / Service
  • -CareSTEP: Transportation Assistance
  • -CareSTEP: Coordinating Care: Dental
  • Advocates for patients and teaches strategies for self-advocacy.
  • May, on occasion, meet a patient in the community to support accessing a needed behavioral health resource or service when the patient needs additional assistance to be successful. Will work with practice manager/direct supervisor to develop a plan and ensure patient needs are met.
  • Provides updated information to the Community Resource Team Lead to ensure the maintenance of up-to-date resource information.
