Job Summary: The Community Resource Specialist I provides high quality customer service and serves as the main point of contact for the organization.

Requirements

  • Excellent communication skills including active listening.
  • Service-oriented and able to resolve patient grievances.
  • Proficient computer skills with the ability to learn new software.
  • Knowledge of, or ability to learn, basic medical terminology and NOAH services.
  • High School Diploma or GED.
  • 1+ years of experience in customer service; healthcare industry experience.
  • New Hires are required to pass pre-employment background check and drug testing (effective 11/1/2022).
  • Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire.

  • Ability to speak English and Spanish preferred.

Responsibilities

  • Schedules and confirms appointments; verifies insurance.
  • Manages referrals, work queues, patient lists, emails, and referrals in Electronic Health Record.
  • Interacts with patients via telephone, email, or in person to provide support and information on NOAH services.
  • Explains community resources and connects patients with appropriate resources.
  • Ensures that appropriate actions are taken to resolve patient problems and concerns.
  • Promotes and provides access to care.
  • Attends, participates, and/or assists in meetings, trainings, community outreach activities, continuing education opportunities, and other activities as required.
  • Performs other related duties as assigned.
