About The Position

The role of the Community Resource Navigator is to provide information and assistance to individuals seeking health coverage, health care and other local resources that aid them in maintaining a healthy life style. They help in educating eligible individuals on how to navigate health insurance options, public programs, and Sea Mar services and assist with the application processes. They aid newly assigned and high risk patients in scheduling their initial appointment for medical, dental and care management services. They provide case management and or participate in intervention strategies that remove barriers that may result in appointment no shows. This service is provided at both Sea Mar service locations or through home visits as necessary. They are responsible for meeting and maintaining all Washington State and Sea Mar testing and certification requirements related to their duties.

Requirements

  • Must sign an oath of permanent confidentiality covering all organizational, customer and patient related information.
  • Will undergo a criminal background check from the Washington State Patrol.
  • Will sign and date the job description to affirm the validity of the information herein, and to signify compliance with the conditions of employment.
  • Travel is required, must have a vehicle and a valid Washington State driver’s license.
  • Must be able to work weekends and evening hours.
  • Proficient in navigating the internet and Microsoft Office programs.
  • Applicant must have excellent customer service skills.
  • They must possess good organizational and time-management skills, demonstrate good judgment, excellent problem-solving abilities and maintain a professional demeanor at all times.
  • The applicant must be proficient at public speaking, spelling, grammar, written communication and the ability to read, interpret, complex documents, reports and data.
  • They must be proficient in Microsoft Office programs.
  • They must be bi lingual, fluent in English and a second language Spanish preferred, with the ability to read and write proficiently in both.
  • Legible handwriting required.
  • This person must be of good character and maintain confidentiality.
  • They need to be self-motivated and have an excellent work ethic, including the ability to work both as part of a team and independently.
  • They must be able to work evenings and weekends as needed.
  • They will be responsible for their specific county assigned but may be required to travel to assist other managed care personal in other counties.
  • Bilingual skills are required.
  • Must be able to communicate effectively with patients and staff in English, Spanish, or Russian, both orally and written.
  • Ability to add, subtract, multiply and divide.
  • The ability to solve practical problems and a variety of situations where only limited standardization exists is required.
  • The applicant must be able to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
  • Either an AA or BA in Social Services or Business Administration with at least 2 years of customer service experience is required, preferably in a health care environment.
  • The applicant must have an initial TB test at hire and an annual retest.
  • This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.
  • Applicants must receive a satisfactory background check from the Washington State Patrol and must maintain a valid Washington Driver’s license, and automobile with liability insurance coverage if hired.
  • Must pass all state required tests and obtain and retain certification by the state related to their position.
  • Pre-hire and annual employee health screening required
  • Annual influenza vaccine required. Only exception is for employees with a medical or religious exemption approved by Administration. Employees with an approved medical or religious exemption must wear a mask at all time during the flu season.
  • While performing the duties of this job, the employee is required to sit for extended periods of time, talk and hear. The employee is required to stand, use hands to fingers, handle or feel and reach with hands and arms. The employee must be able to lift/move up to 50 pounds. The employee is occasionally required to climb or balance and stoop, kneel, crouch or crawl. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Responsibilities

  • Pro-actively aid in increasing public awareness regarding health coverage enrollment opportunities by planning and conducting outreach activities within the community, participating at both Sea Mar sponsored and non-Sea Mar sponsored events.
  • Assist in the development and distribution of informational/promotional materials, and establishes and maintains referral relationships with local organizations, agencies, institutions and businesses as directed by the Managed Care Department.
  • They will routinely work uninsured lists from Sea Mar dental, behavioral health, and ancillary service sites to advance enrollment of eligible individuals onto health coverage and inform non eligible individuals about health care options.
  • Assist consumers with online public health coverage and program applications, provide the necessary information for consumer to navigate health plan options and make well informed decisions.
  • Serve as a service navigator for consumers wishing to establish Sea Mar as their medical home, by providing information regarding services available, and consumer rights and responsibilities.
  • Be responsible for assisting high risk patients in scheduling a care manager appointment and facilitate processes to ensure they’ve established care. In using a case management model incorporating home visits if necessary to assist patients who fail to keep their initial medical visit by removing impediments by identifying and linking them to necessary resources.
  • Be responsible for maintaining daily activity records, submitting progress reports in a timely manner, and assist in identifying areas for quality improvement and efficiency to the Managed Care Department.
  • Assists in identifying, monitoring and tracking uninsured individuals and advocating on their behalf regarding barriers to coverage and or access of health care services.
  • Performs all duties in a professional manner, participates at all mandatory state and corporate trainings and meet all competency testing requirements, on health coverage policy changes impacting consumers and or processes.
  • Perform any and all other duties as may be assigned while adhering to all corporate and departmental policies and procedures.
