JOB DESCRIPTION AND POSITION REQUIREMENTS Scheduled Weekly Hours: 40 Hours: Monday through Friday 8:00 am - 4:30 pm JOB SUMMARY: The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and helps coordinate a plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members.

Requirements

• LMSW required – must be under supervision for LCSW; LCSW preferred.
• Working knowledge/experience in case management, payors and reimbursement.
• Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
• Organized and efficient in establishing priorities and performing tasks within specific timeframes.
• Self-motivated requiring minimal supervision.
• Views problems as a challenge and investigates alternatives which demand creativity, flexibility and the ability to work under stress.
• Responsible for maintaining strict confidentiality of all verbal & written information and documents.
• Familiarity with and appropriate use of community resources.

Nice To Haves

• LCSW preferred.
• Experience in psychosocial and therapeutic counseling preferred.
