About The Position

The Community Relations Coordinator supports the clinic(s) by implementing the Community Relations Department's objective to carry out CHSI's mission through developing community partnerships and relationships, organizing clinic resource events, and educating the community about healthcare system navigation, health education, and social determinants of health.

Requirements

  • Strong analytical and critical thinking skills; ability to pay close attention to detail.
  • Strong computer skills, with proficiency in Microsoft Office
  • Well-developed verbal and written communication skills and the ability to work cooperatively with divergent and diverse groups
  • Must possess good customer service skills, including verbal and written communication.
  • A high standard of professionalism is expected in speech, manner, attitude, and appearance at all times.
  • Possess strong interpersonal skills and ability to work well with others.
  • High standard of professional ethics and conduct is expected.
  • Ability to deal effectively with changing situations and stressful environments.
  • Ability to perform tasks related to physical activity to complete the position's responsibilities.
  • Be able to lift up to 50 lbs. with the help of another person.
  • Three years or more of related experience is required.
  • Current CA Driver's License and proof of automobile insurance
  • Must be able to obtain a Certified Enrollment Counselor (CEC) certification within 60 days of employment
  • Must be able to travel locally 60% of the time

Nice To Haves

  • Bilingual (English/Spanish) strongly preferred
  • Bachelor's Degree or five years of experience is preferred.

Responsibilities

  • Upholds and supports the mission, philosophy, objectives, policies, and ethics of CHSI.
  • Will smile and speak courteously and friendly with patients, staff members, and clients.
  • Protects the organization's value by keeping information confidential.
  • Provide timely feedback to the direct leadership and work in collaboration with the clinic Site Manager to implement the Community Relations Department's specific strategic plan to increase awareness and health education on healthcare system navigation.
  • Meets regularly with the Site Manager and designated staff to help identify and/or communicate community issues, feedback, service needs, events, and the Community Relations Department's strategic plan.
  • Comply with expectations of grant requirements for the purpose of statistical tracking to generate monthly reports.
  • Serve as a liaison between the Community Relations Department, health center staff, and the community while continuing contact with social networks and community partners.
  • Assist the Community Relations leadership in establishing venues to generate community partnership opportunities through existing and new accounts, cold calling, and educational presentations.
  • Ability to find resources to assist patients with needs outside the scope of our services at CHS.
  • Acts as a liaison in performing value-added activities such as community events, presentations, customer service, and service recovery to ensure patient satisfaction.
  • Continuously update the designated staff who administers CHSI social media educational campaigns.
  • Responsible for participating in quality improvement projects/activities and accountable for quality and PCMH.
  • Host community events and attend community events representing CHSI.
  • In-reach: Deliver in-office/clinic patient contact services to raise awareness about the organization's programs and services. Facilitate patients' education about healthcare options for uninsured patients.
  • Outreach: Provide patients with contact services outside of the clinic to promote the organization's programs and services.
  • Attend key community meetings and events as a representative of the assigned clinic to network and build contacts.
  • Perform all other duties as directed, either formally or informally, verbally or in writing.
