About The Position

The Community Program Liaison supports NOVUS Health’s outreach and community engagement efforts while serving as a connection between patients, community members, and available resources. This role focuses on organizing and participating in outreach events, promoting NOVUS Health programs, and engaging directly with the community to increase awareness of services. The Community Program Liaison will provide occasional support to the NOVUS food pantry and related programs as needed.

Requirements

  • High school diploma or GED required; associate or bachelor's degree in social work, public health, human services, or a related field preferred.
  • Valid driver’s license and clean driving record.
  • 1–2 years of experience in community outreach, social services, health educator, or other community-based practitioner. Experience working with diverse populations, especially in underserved communities.
  • Strong interpersonal and communication skills, ability to engage compassionately with patients, families, and community partners.
  • Problem-solving skills
  • Cultural competence; understanding of and sensitivity to the needs of diverse populations.
  • Basic computer proficiency for data entry, email communication, and using internal systems.
  • Ability to lift and transport items (up to 40 lbs.).
  • Comfortable driving agency or personal vehicle for events, pickups, deliveries.
  • Willingness to work flexible hours, including evenings and weekends.

Responsibilities

  • Participate in community outreach events such as health fairs, neighborhood events, and partner organization activities.
  • Assist with outreach event preparation including organizing materials, transporting supplies, and setting up outreach tables or booths.
  • Represent NOVUS Health by tabling at events, engaging with community members, and sharing information about available services and programs.
  • Assist with breakdown and clean up following outreach events and ensure materials are returned and stored properly.
  • Maintain familiarity with local community resources that support health, wellness, and stability.
  • Provide occasional support to the NOVUS food pantry and related programs as needed.
  • Assist patients or families in completion of food program intake/survey and guide them in selecting food from pantry.
  • Support food pantry operations including organizing inventory, monitoring stock levels, and checking expiration dates.
  • Assist with picking up food donations as needed.
  • Drive agency vehicle (or personal vehicle when approved) to transport outreach supplies, materials, or donations to and from events or partner locations.
  • Assist with loading and unloading outreach materials, supplies, and donations.
  • Ensure outreach materials and equipment are organized, stocked, and ready for events.
  • Participate in departmental meetings, staff meetings, and trainings as assigned.
  • Exhibit excellent working relationships with patients, visitors, community members, and staff.
  • Effectively communicate NOVUS Health’s mission, services, and programs.
  • Maintain knowledge of community resources appropriate to the needs of patients and families.
  • Maintain HIPAA compliance and protect patient confidentiality.
  • Work flexible hours as needed to support outreach events, including evenings or weekends.
  • Other duties as assigned.
