Gaudenzia's Mission: Gaudenzia's Mission is to promote long-term recovery by providing comprehensive treatment and support to people with substance use disorder, co-occurring mental health disorders, and related conditions, regardless of individual circumstances. This individual is responsible for providing case management services and will be trained to conduct Recovery Management Checkups (RMCs). The CHW will utilize the SAMHSA approved Relapse Prevention Therapy curriculum and Wrap Around/PBRSS that includes but is not limited to transportation; employment services/job training; community case ma,1agement to provide coordination and linkages with other services; referrals; life skills training and education on SUD/COD. The CHW is trained to lead New Client Orientations and psychoeducation groups designed to improve access to and retention in MAT and facilitate long-term recovery. Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving obj_ectives.
