About The Position

The Community Health Worker under the PHA CARES program will provide CHW services primarily to original and new residents in Bartram Village, a Philadelphia Housing Authority development. The CHW will conduct intake and assessments, plan health education workshops virtually or in-person at least monthly, may be asked to do home visits and make phone contact, maintain data, and contribute to quarterly reports on outcomes. Interaction between the CHW and PHA residents is designed to promote positive behavior change, regular health monitoring, and stress the importance of addressing health issues for themselves and other family members.

Requirements

  • High School Diploma or Equivalent and 6 months of relevant experience. An equivalent combination of education and experience may be considered.
  • Strong computer-related skills using Zoom, Microsoft Teams, Microsoft Office (Word, PowerPoint, Excel), and Google applications.

  • Resident of Philadelphia Housing Authority (PHA)
  • PA Child Abuse Clearance

Nice To Haves

  • Completion of a minimum of 75 hours of Community Health Worker Core Training
  • General Experience in a medical or health-related field and knowledge of medical terminology
  • Resident of PHA Bartram Village location
