Community Outreach Navigator – St. Joseph Warren Hospital Primary Function/General Purpose of Position The Community Outreach Navigator participates in community meetings, supports patients and families as they navigate the healthcare system, and provides direct patient care under the guidance of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health.

Requirements

  • Bachelors, Social Service or healthcare related field (required)
  • 1 year in healthcare, social or human services setting (required)
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
  • Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.

Nice To Haves

  • Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or other social service or healthcare related licensure (preferred)
  • Masters, Social Service or healthcare related field (preferred)

Responsibilities

  • Participates in treatment team meetings as appropriate.
  • Participates in community and committee meetings.
  • Develops working relationships with community providers.
  • Community resource knowledge and navigation; coordinates wrap around services for complex patients.
  • Identifies program barriers and future program needs.
  • Completes social determinants of health (SDOH) screenings, substance use disorder (SUD) screenings, Columbia-Suicide Rating Scale (C-SSRS)
  • May complete patient health questionnaire (PHQ-9), generalized anxiety disorder (GAD), post-tramatic stress disorder (PTSD) assessments and/or screenings (within scope)
  • Successfully participates to complete BSMH and departmental specific training as required
