About The Position

The Community Outreach Coordinator is responsible for supporting Tapestry 360 Health (Tap 360) patient growth initiatives by facilitating access to services. They develop and strengthen relationships with existing and potentially new clients, patients, elected officials, and other community representatives to engage with new patients and help them navigate healthcare services. In addition to supporting a variety of outreach initiatives to build patient volume throughout our service area, they will work as part of a multidisciplinary team to increase awareness about substance use disorder, with an emphasis on opioid use, create an education campaign, and connect potential patients to the Medication Assisted Treatment (MAT) Program.

Requirements

  • Associate degree in mental health, community health, or another related field
  • Two years of experience in health education, case management or community engagement on Chicago’s North Side required.
  • Proficient in Microsoft Office: Excel, Word, Publisher, and PowerPoint.
  • Demonstrated ability to effectively work independently and as part of team.
  • Demonstrated problem solving and critical thinking skills.
  • Ability to build respectful and trustful relationships with people from different backgrounds and lived experiences.
  • Knowledge about current immigration issues.
  • Excellent written & interpersonal communication skills.
  • Ability to communicate using plain language.
  • Compassionate, empathetic, proactive, resourceful, gregarious, and amiable.

Nice To Haves

  • Prior experience in project management, highly preferred.
  • Prior lived experience or experience with addressing substance use disorder is a plus.
  • Public speaking and social media management experience.
  • Bilingual and/or bicultural highly preferred.

Responsibilities

  • Assist in developing and implementing strategies to enroll community-based patients in services through virtual or in-person activities such as meetings, one-on-one visits, school events, community health fairs, presentations, referral agreements and distribution of materials to community partners.
  • Research and analyze data to identify and define referral sources and potential patients.
  • Promote a holistic approach to care and connect individuals to primary care services, mental health services, integrative medicine, etc.
  • Serve as a liaison between community members and Tap 360 to evaluate the quality of our services and address barriers to accessing care such as transportation, navigation, proximity, hours of operation, and cultural barriers such as stigma, immigration, religion, etc.
  • Facilitate appointment scheduling for MAT program participants and new patients while conducting outreach events.
  • Collaborate with MAT team to create a multilingual plain language campaign to educate our service areas about substance use disorder, including overdose prevention, access to substance use services, treatment options, etc.
  • Procure partnership with potential employers/employment training agencies for MAT program participants.
  • Track data and generate reports as requested by supervisor, funders, and governmental agencies.
  • Evening and weekend availability, with advance notice, is required.
  • Other responsibilities as assigned.
