About The Position

CCAP Mission Statement - To empower all people and communities, challenged by poverty as well as social and cultural barriers, through advocacy, education, and access to high quality health and human services. JOB SUMMARY: The Community Outreach Case Manager will be responsible for completing assessments with consumers from targeted area who request assistance related to human services. The case manager will increase awareness while connecting vulnerable individuals with agency services and statewide resources. The case manager will also be responsible for fostering relationships with community partners at locations where the case manager will be housed. The case manager will provide follow up with resources provided to ensure connection. WORK SCHEDULE DEMANDS: This is a full-time 35 hours per week position. Monday - Friday. Due to the nature of the job, this individual must be flexible with their schedule to accommodate the client population. May be required to work at any CCAP location.

Requirements

  • Associate’s degree in human services, family development, or related program/field.
  • Experience in community outreach, case management, and data management.
  • Knowledge of statewide resources.
  • Ability to work independently and coordinate multiple tasks.
  • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications and the Microsoft operating system.
  • Must have excellent oral and written communication skills for all levels of client and colleague interaction.
  • Provide/maintain all required immunization and /or vaccines.
  • Complete all required background checks.
  • Valid driver’s license and active vehicle auto insurance.

  • Bachelor’s degree, preferred.

Responsibilities

  • Conduct outreach to targeted customers around the state, ensuring the effective and meaningful engagement of community members seeking services.
  • Provide information about and access to basic human need services/resources in the communities served.
  • Work with social service staff to link customers to services within social services.
  • Collaborate with staff from targeted communities’ libraries and senior centers.
  • Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness.
  • Utilize all CCAP guides, social service resource manuals, Unite Us system and United Ways 211 to maximize resource awareness for linkages and resources.
  • Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Responsible for collecting consumer information and accurately entering it into Codect system.
  • Other duties as assigned.
