This position may work in a Designated Healthcare Facility which is subject to the State of Maine’s immunization requirements for healthcare workers. If hired, you will be required to show a Certificate of Immunization, or Proof of Immunity, against: 1.) Rubeola (measles); Mumps; Rubella (German measles); Varicella; Hepatitis B; and Influenza. Schedule: Full-Time, Monday-Friday, Typically 7:30am-4pm | Some schedule flexibility may be required for coverage needs. (This position requires regular travel between PCHC sites . Mileage for any travel between sites will be reimbursed in accordance with PCHC’s Business Travel & Expense Reimbursement policy.) Salary: Ranges from $21-30/hour depending on qualifications, skills, and years of relevant experience. Coordinate Care for Complex Needs: Recognize and respond to chronic conditions, connecting patients to Care Management, Transitions of Care, and other supportive services as needed. Engage Patients & Families with Compassion: Provide education on chronic disease management, preventative care, and advance care planning—ensuring patients and families feel informed and supported. Strengthen Workflows & Quality Outcomes: Partner with Operations, Quality, and clinical teams to support consistent workflows, meet CMS requirements, and drive performance in Annual Wellness Visits and ACO initiatives Work Across the Community: Travel to local care settings to provide on-site support, building strong relationships with patients, families, and care teams while expanding access to essential services. Education and Experience: Medical Assistant certification required, as below: AAMA - Certification by the AAMA required at time of hire or within 60 days of date of hire. Certifications must be maintained at all times. CCMA- Certification by the NHA required. Certifications must be maintained at all times. Minimum of two years’ experience as a Medical Assistant in primary healthcare or related setting required. Will have up to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross, or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire. Having a valid driver’s license with an acceptable driving record and/or an appropriate length of driving experience for insurance purposes is required. Any concerns raised by the results of a driving record check will be subject to an individualized review to determine whether the infraction is relevant to the individual’s ability to perform the responsibilities of the position in accordance with PCHC’s Automobile Safety and Background Check Policy.
