About The Position

As a Community Living Coordinator, you will lead the delivery of high-quality, person-centered care for individuals with IDD. You will be responsible for developing individualized service plans, conducting home visits, and overseeing our direct care teams—from hiring and onboarding to ongoing performance coaching. This role requires a resilient, well-reasoned problem-solver capable of navigating complex social service challenges, managing urgent crises, and ensuring strict regulatory compliance. By fostering collaborative relationships with families and stakeholders, you will serve as a vital advocate for the health, safety, and independence of those living within our community programs.

Requirements

High school diploma or equivalent
One year of experience working with the IDD population or in a similar social service or healthcare setting (Required)
A valid driver's license and reliable transportation.
The ability to pass comprehensive background checks per state and company regulations/policies

Nice To Haves

Bachelor's degree (Preferred)
Prior experience as a supervisor/manager in a healthcare setting (Preferred)

Responsibilities

Conduct bi-monthly in-person home visits to provide support and ensure the quality of shared living services.
Oversee and manage Direct Support Professionals (DSPs).
Collaborate with individuals with IDD, their families, and other professionals to develop individualized support plans.
Advocate for individuals' needs and connect them with community resources.
Motivate and support your DSP team to deliver exceptional care.
Maintain accurate documentation and ensure regulatory compliance.
