About The Position

For nearly two decades, Health Link Home Health & Hospice has provided exceptional home health and hospice services throughout the San Francisco Bay Area—delivering compassionate, high-quality care where patients are most comfortable: at home. As we continue to grow, we’re looking for a driven Community Liaison to help expand our reach and strengthen trusted community partnerships. Home Health is built on trust. Our referral partners trust us with their patients because we keep our promises and live our values. In this role, you’ll help ensure more patients gain access to our outstanding clinicians by building, maintaining, and deepening relationships across the healthcare community. As a Community Liaison, you will work closely with our Community Liaison Team and Director of Community Relations to execute sales and marketing strategies that support agency growth. You’ll serve as the key connection between referral partners and internal teams, ensuring smooth communication, strong follow-up, and consistent service delivery. Prior home health or hospice experience is a plus—but not required. What is required is curiosity, adaptability, strong relationship-building skills, and the ability to learn quickly in a fast-paced environment. Though Health Link has nearly 20 years of community trust behind us, we still operate with a startup mindset. Responsibilities

  • Support and execute Health Link’s sales and marketing strategy
  • Develop and maintain relationships with hospitals, physicians, clinics, SNFs, senior living communities, and other referral partners
  • Conduct ride-alongs with team members to respect existing relationships while developing your own
  • Schedule meetings, conduct in-person visits, and manage consistent follow-up
  • Collaborate closely with intake and clinical teams to coordinate referrals
  • Represent Health Link at community events and networking opportunities
  • Track and manage an individual budget and related expenses
  • Maintain excellent responsiveness, calendar management, and CRM documentation
