Position: Home Health and Hospice Community Liaison The Home Health and Hospice Community Liaison plays a key role in driving growth by developing strong relationships with referral partners, educating the community about hospice services, and representing Sequoia Hospice with professionalism and compassion. This role requires a balance of healthcare knowledge, outreach, sales strategy, and communication.

Requirements

  • Bachelor’s degree in Marketing, Business Administration, or related field preferred (experience may substitute).
  • Minimum 3 years of healthcare marketing or community outreach experience, preferably in hospice or home health.
  • Strong skills in relationship building, communication, public relations, and negotiation.
  • Ability to market effectively, work independently, and collaborate with clinical and operational leadership.
  • Demonstrates organization, initiative, professionalism, and adaptability.
  • Knowledge of healthcare business operations, referral development, and post‑acute care environments.

Responsibilities

  • Build and maintain relationships with physicians, discharge planners, case managers, social workers, hospitals, skilled nursing facilities, assisted living communities, and other referral sources.
  • Conduct onsite visits to identify patients appropriate for hospice services and promote Sequoia Hospice as a trusted provider.
  • Stay current on industry trends, hospice eligibility criteria, compliance standards, and post‑acute care best practices.
  • Represent Sequoia Hospice at health fairs, community events, outreach programs, and professional networking functions.
  • Conduct market assessments and develop targeted marketing strategies to achieve census and growth goals.
  • Collaborate with the Executive Director on marketing initiatives and business development techniques.
  • Promote Sequoia Hospice as a high‑quality, patient‑centered provider in the community.
  • Identify new opportunities for service enhancements and expansion.
  • Maintain working knowledge of key payer sources, managed care organizations, government programs, and competitor landscape.
  • Assist patients, families, and referral sources in accessing community resources when services fall outside agency offerings.
  • Track, report, and evaluate the cost-effectiveness of marketing efforts.
