About The Position

JOB SUMMARY Responsible for assigned functions related to providing information to the public, community events, identified referral sources, groups, and individuals. Works to promote the RVHHH mission consistent with agency policies and procedures.

  • Minimum of four (2) years of varied work experience in related field

  • Ability to travel within the agency’s service area of 1.5 hours of driving distance of your home office
  • Continuing education for professional growth

Responsibilities

  • Identify audiences and determine the best way to communicate home health, hospice, Palliative Care/ end-of-life information and services clearly and efficiently for the intended audience
  • Serve as staff liaison for RVHHH with physicians, hospitals nurses, social workers, discharge planners and community agencies
  • Follows-up on referral for evaluation and/or admission to Hospice and Home Health
  • Responsible and/or overseeing the process of hospice orders/Home Health orders getting to the Physicians for signature in a timely manner
  • Work with various organizations to facilitate the delivery of appropriate services for potential home health and hospice patients.
  • Perform community outreach activities including presentations, exhibits and one-on-one contact with community groups, Physicians, and coordinate special events
  • Collaborate with healthcare specialists and civic groups to determine community home health, hospice, and Palliative Care/ end-of-life needs and availability of services to maintain continuity of care for patients and families in the areas we serve
  • Provide current information regarding agency services including preparing, creating, and distributing printed home health, hospice and Palliative Care / end-of-life health education materials with final approval from the DON/DPCC
  • Promote use of RVHHH's home health program and services
  • Develop a dynamic communication method for increasing the utilization of our hospice services within various established and non-established referral sources
  • Educate the community regarding hospice care and home health services available with RVHHH
  • Attends and participates in staff meetings, in-services, and case conferences
  • Provide information to team members in written form or in-person regarding home health and hospice information, education and services communicated within the community.
  • Responsible for maintaining continuity of messaging by sharing all written or word-of-mouth marketing, educational, promotional materials with all team members involved in the delivery RVHHH home health, hospice, and Palliative Care / end-of-life services.
  • Participates in continuing education through academic studies, seminars, and workshops
  • Completes all documentation in an accurate and legible manner
  • Attend IDT, as required.
  • Collect data for Admission activities
  • All other duties as assigned by the DON/DPCCC
