About The Position

Colorado PACE has an immediate opening for a Community Liaison in Aurora, CO. This full-time role operates Monday-Friday, 8-5. The Community Liaison is responsible for building and maintaining community partner relationships to increase referral volume, the number of PACE participant enrollees, and expand market share. This involves providing information and promoting PACE services through campaigns to various referral sources such as affordable housing, independent living, community- and faith-based organizations, hospitals, assisted living facilities, and skilled nursing facilities. The role also includes building effective relationships within assigned accounts, utilizing and providing input on marketing campaigns, and providing effective outreach and education to achieve strategic goals. The liaison will keep detailed records of outreach activities, cultivate new relationships, and collaborate with marketing and clinical teams on strategies. They will also work with key stakeholders to address referral barriers, educate the community on PACE benefits by attending events, and assist with assessing service delivery and enrollment activities when required. Care Synergy is a network of mission-driven, nonprofit healthcare organizations dedicated to caring for very ill, frail, and primarily elderly individuals by keeping them at home and out of the hospital while maximizing comfort, dignity, and quality of life. The Care Synergy enterprise includes a group of collaborating nonprofit Affiliates across Colorado’s Front Range that share resources, expertise, and infrastructure to deliver high quality, compassionate care. Collectively, the enterprise generates approximately $100 million in annual revenue and serves thousands of individuals and families each year through hospice, home health, palliative care, CAPABLE, wellness, and PACE programs.

Requirements

  • Minimum Education: High School Diploma. Some college required.
  • Minimum Experience: Four to five (4-5) years’ experience in marketing, sales, or healthcare relationship management.
  • Required License: Valid Colorado state driver’s license and current auto insurance.

Nice To Haves

  • Bachelor’s degree in Marketing, Public Relations or healthcare-related field preferred.
  • Experience in lieu of education may be considered.

Responsibilities

  • Builds and maintains community partner relationships and leverages those relationships to increase referral volume, number of PACE participant enrollees and expands market share in the community we serve.
  • Provides information and promotes PACE services through campaigns to grow referral sources including but not limited to: affordable housing, independent living, community- and faith-based organizations, hospitals, assisted living facilities, and skilled nursing facilities.
  • Builds effective relationships within assigned accounts i.e., affordable housing, Single Entry Point, community- and faith-based organizations, Long-Term Care/Assisted Living facilities, physician offices, home health, and other partners to promote communication, problem-solving, and opportunities to position Colorado PACE as the preferred organization.
  • Utilizes and provides input with regards to marketing campaigns and associated tools to build brand awareness.
  • Provides effective outreach and education to achieve strategic goals and maximize market potential.
  • Keep detailed records of outreach activities and create a schedule of future actions.
  • Cultivates new relationships and referral sources.
  • Collaborates with Marketing and clinical teams to create marketing strategies to increase appropriate and timely referrals.
  • Works with all key stakeholders to understand and address identified barriers to referrals.
  • Educates community on the benefits of PACE for participants, family, and caregivers by attending community events, trade shows and other public events as well as attendance at meetings and company functions.
  • Assists with assessing our service delivery with referral sources through periodic surveys, utilizing report information and conveying recommended changes to processes and programs as appropriate.
  • Assists with enrollment activities when required.
