The Community Liaison functions as an integral member of the Community Programs team under the umbrella of the Office of Medical Education to support all community immersion components of the Human Dimension (HD) Course, namely the Human Dimension Voices Program (HDVP).This role partners with pairs of medical students as they are matched with individuals and families in the community as part of the core curriculum of their medical education. This position is based out of our campus in Nutley, NJ. This is a hybrid role, requiring 3 days on site per week. Additionally, this role requires travel across communities in northern NJ. The standard work schedule for this role is 9:00 am - 4:45 pm, but there are occaisional shifts that may start as early as 8:00 am, based on needs for the academic program. Notice will be given for days where flexibility is needed.

Requirements

  • Bachelor degree in Social Work, Public Health, or Related Field or a High School diploma, general equivalency diploma (GED), and/or GED equivalent programs and a minimum of 4 years of related experience.
  • Experience working with community based organizations.
  • Ability to assess an environment for safety from both a physical and emotional standpoint.
  • Strong interpersonal skills.
  • Analytical and evaluation skills.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
  • Valid Driver's License from a USA state.

Responsibilities

  • Recruits families via community outreach activities and other avenues to participate in the SOM`s Human Dimension course.
  • Supports team`s community engagement events on campus and in the community.
  • Provides mentorship, guidance to medical students` regarding their relationships with program participants including monitoring of visits and support with relationship building and resource finding in order to support student learning and ensure retention of program participants.
  • Assists School of Medicine Faculty to support students.
  • Builds relationships with community agencies to analyze and determine services that SOM can provide within the community.
  • Makes connections between community residents, supports advocacy efforts, and provides feedback on the implementation of the HD course`s community immersion efforts. serve as a continuity point of contact for community members.
  • Partners with the Community Outreach department within Hackensack Meridian Health to determine appropriate opportunities for the medical students and resources for the families they work with in local communities.
  • Reviews and analyzes population health information to contribute to the overall decision- making and direction of the HD program.
  • Analyzes the population`s access to healthcare and the community demographics to ensure the HD program and its students are enhancing population health initiatives.
  • Conducts phone interviews and in-home safety screenings to ensure medical students are being placed in a safe environment.
  • Ensures all logistics related to safety screenings and agency needs are completed in a timely, efficient and accurate manner.
  • Recommends outreach activities to address identified community health needs; including orientations, community engagement sessions, trainings and presentations.
  • Other duties and/or projects as assigned.
