POSITION SUMMARY The Liaison’s primary goal is to grow Cooperative through acquisition of new clients and the expansion of services to our current ones. While a Community Liaison understands and sells home care services funded by the state, their primary focus is on Private Services. This person should have a comprehensive understanding of Cooperative Home Care including all services and funders, and their presentation of this information should be consistent with how Cooperative Home Care presents itself online and in all other marketing resources. Requirements

  • Education (Required): Four-year degree

Nice To Haves

  • At least 2 years sales experience in home care and/or health-related field
  • Previously established connections with key referral sources
  • Knowledgeable of and compliant with Medicare and insurance regulations
  • Currently licensed practical/registered nurse

Responsibilities

  • Identify strategic community events and partnership opportunities to assist with the growth of the Home Health division
  • Understands all Company funding sources
  • Have a comprehensive understanding of the assigned territory and be adept at identifying both current and prospective business opportunities.
  • Evaluate and ranks referral accounts based on strategic value and proactively seeks opportunities to enhance referral potential where appropriate.
  • Responsibly manage all assigned marketing collateral
  • Attend quarterly strategic partnership meetings.
  • Solicits referrals and builds strong relationships between Cooperative and referrals sources.
  • Adapts their selling methods as needed to be as effective as possible
  • Creates seamless hand-off to Home Health Team, making timely introductions, communicates thoroughly with referral sources and internal team, and checks back at key interval(s).
  • Acts as a liaison between clients and Cooperative Home Care clinical staff to resolve problems, provide up to date information, and to maintain positive customer relations.
  • Visit virtually and in-person with physicians, discharge planners, estate planners, and other related health care providers regarding service provisions, contracts and communication with Cooperation.
  • Creates and completes community education presentations, virtually and in-person, to current and prospective clients to educate them on Cooperative’s services.
  • Understands and monitors Medicare’s Home Health Compare scores and contributes to improving Cooperative’s scores.
  • Promotes positive relationship between Cooperative Home Care and Department of Health and Senior Service, Department of Social Services, Department of Mental Health, School Boards, and any other contract held by Cooperative Home Care.
  • Maintains networking base in the community through trade associations, business organizations and local association meetings (virtual and in-person).
  • Collaboratively communicates/consults with the licensed professional on any referrals received and follows through with the Clinical/Case Managers and or Client Service Supervisors as needed.
  • Attends meetings, health fairs, and other in-person or virtual events as requested by the CEO and/or Executive Director.
  • Participates in mutual KPI/goal setting, ongoing understanding of KPIs and strengths/areas if improvement.
  • Projects professional attitude/appearance toward agency staff, referral sources, and general public online.
  • Maintains confidentiality of patient, employee, and Company information.
  • Submits documentation within required timeframes.
  • Ability to write, dictate, or use a keyboard to communicate directive effectively and clearly.
  • All other tasks as assigned.
