About The Position

This position will be split hours between our Day Hab Services and our Supported Employment Services. This role operates in a Day Habilitation (DAY HAB) Setting, defined as services “that provide opportunities and support for community inclusion and build interest in and develop skills for active participation in recreation, volunteerism and integrated community employment.” (Iowa Health & Human Services). Day Habilitation provides assistance with acquisition, retention, or improvement of socialization, community participation, and daily living skills. This role operates in a Supported Employment (SE) Setting, defined as services “that enables the member to obtain and maintain individual employment” (Iowa Health & Human Services). Rather than assuming individuals with development or intellectual disabilities are not qualified to work, Supported Employment services provide these individuals with the opportunity to obtain and maintain meaningful employment that supports their life goals. Community Integration Professionals support the mission of the organization – empowering people to reach individual achievement across the spectrum of life. As an integral part of the overall supported employment team, you will collaborate with other Community Integration Professionals, Direct Support Professionals, Programmers, Supervisors, individuals-served, and their families to ensure the best possible care for those you serve. You will creatively strategize ways for an individual to find employment that suits their skills, abilities, and goals. Your innovative and impactful solutions and strategies will make the difference for those you serve.

Driver’s License
Reliable Transportation
Automobile insurance covers $100,000/$300,000/$50,000 liability coverage
High school diploma or the equivalent representing their high school education
Ability to work in the U.S. without sponsorship
May require lifting at times, up to 35 pounds
Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment
May require prolonged sitting and working at a computer for up to eight hours
May require frequent bending, stooping, and stretching
May require prolonged standing for up to eight hours
Must wear hearing and eye protection in required areas
Must have ability to see objects/persons at a distance
Must have ability to see close work such as typed or handwritten material
Must have ability to hear conversations in a quiet and a noisy environment
Must have ability to determine where a sound is coming from
Must have ability to hear differences among bells, buzzers, beeps, horns, etc.
Must have ability to communicate through speech
Must have ability to drive and transport for service provision if needed
Must remain awake during working hours, if applicable

Responsibilities

Collaborate with the individual to create meaningful daily activities within their community based on their skills, abilities, and goals.
Identifying the members’ interests, preferences, skills, strengths and contributions.
Planning and coordination of the members individualized daily and weekly day habilitation schedules.
Participating in community activities related to hobbies, leisure, personal health, and wellness.
Participating in community activities related to cultural, civic, and religious interests.
Participating in adult learning opportunities and volunteer opportunities.
Collaborate with the individual to create a plan that provides them with meaningful employment within their community based on their skills, abilities, and goals.
Provide services to individuals based on their unique goals and behavioral care plan.
Teaching individuals how to accomplish their goal (rather than completing a task for them).
Using behavioral strategies that support individuals and address any behaviors that do not lead to their success.
Motivate and encourage individuals to participate in their behavioral care plans and reach their goals.
Participate in recreational, community, and social activities with those you serve, honoring the choices of the individual.
Display and understanding of when it is appropriate to apply rights-restrictions, only as determined by the interdisciplinary team and when indicated in the individual’s behavioral care plan.
Provide coaching responsibilities, acting as a liaison between Imagine the Possibilities and the opportunities available to individuals.
Partnering with schools, local businesses, and local organizations to ensure the individual is set up for success.
Connecting individuals with experts in their community in the fields of education, recreation, spiritual development, employment, cultural missions, or civic needs.
Provide complete, consistent, and accurate documentation of the individual’s progress.
Provide safe environments and instruction to individuals served regarding the practices that will help them reach their goals.
Be an advocate for individuals receiving services to have as much control over their own lives as possible.
