In this role you will assist and support clients individualized skill development, independent living and community integration so individuals can learn how to actively and independently engage in their local community. This is a full-time position that typically works Monday-Friday during normal business hours but may require some need for occasional nights/weekends-based business needs. Provide community inclusion support either to individuals or small groups- up to three clients- who have similar interests or goals. Ensure that all services provided are consistent with the community inclusion plan and must include a specific goal and information about the client's skills, interests, gifts and preferred activities. Assist the client with locating and initially engaging in activities that support socialization, education, recreation and personal development for the purpose of building and strengthening relationships with others in the local community who are not paid to be with the person learning, practicing and applying skills that promote greater independence and inclusion in their community. Safely operate a personal vehicle to travel between work sites, or attend off-site meetings, as required. ADDITIONAL RESPONSIBILITIES: Performs related duties as assigned.
