About The Position

Under the direction of the Community Inclusion Supervisor, the Community Inclusion Coordinator will act as the liaison for the client and the community. Community Inclusion Services are individualized and provide clients with opportunities to engage in community-based activities that support socialization, education, recreation, and personal development. The Community Inclusion Coordinator will interact with program participants/clients and must be familiar with the County Guidelines issued by the Developmental Disabilities Administration. At all times, the Community Inclusion Coordinator will seek to act in accord with the principles and values expressed in the guidelines and uphold the agency mission "to serve individuals with developmental disabilities by providing an array of residential and other support services that will result in maximizing the person’s quality of life." The Community Inclusion Coordinator will work the shift assigned by the supervisor.

Requirements

  • At least one year of experience supporting adults with developmental disabilities OR two years of working within a social service program
  • At least 18 years of age
  • A high school diploma or GED
  • Intermediate computer skills, including knowledge of Microsoft Office Suite and the ability to learn and use new software
  • Washington State Driver’s License, current vehicle insurance, and ability to be covered on Alpha Supported Living Services’ automobile insurance plan based on his/her driving record
  • Background inquiry clearance from an authorized state agency

  • Knowledge of human behavior, including individual differences in ability, personality, interests, and motivation, as well as assessment and treatment of behavioral and affective disorders
  • Skill in motivating, developing, and directing individuals in their work, with a focus on identifying the most suitable candidates for specific roles or activities
  • Ability to serve as a liaison between clients and the community

Responsibilities

  • Conduct assessments to determine clients' skills, interests, and potential for community involvement through interviews and research
  • Provide personalized support to Community Inclusion clients during their outings into the community
  • Prepare and maintain comprehensive six-month progress reports on Community Inclusion Plans
  • Accurately document time spent on behalf of each client, ensuring adherence to the requirements of the Developmental Disability Administration Community Inclusion Billable Activity
  • Manage a caseload of approximately 8-9 clients per week, adjusting schedules according to client availability and monthly allotted hours
  • Drive clients to and from activities, adhering to scheduled appointments and commitments
  • Assist with personal care tasks as needed, such as restroom assistance
  • Utilize behavior management plans to provide positive redirection for clients with mild behavior issues
  • Maintain thorough documentation of all outings and related work activities for billing purposes
  • Update and maintain client care plans every six months, and attend annual assessment meetings to report on client progress
  • Serve as a bridge to help clients connect with their communities and facilitate meaningful interactions
  • Contribute to the mission of maximizing the quality of life for individuals with developmental disabilities through dedicated service.
