About The Position
At New Horizon Counseling Center, we meet people where they are — literally. As a Community Outreach Specialist, you’ll connect with individuals and families in everyday spaces like laundromats to help them access vital supports such as SNAP, Medicaid, public assistance, and healthcare services such as primary care, and outpatient behavioral health. Your work will reduce barriers and bring resources directly to the community.
Requirements
- Comfort initiating conversations with strangers in community settings
- Passion for helping underserved communities access essential benefits
- Empathy and Rapport: The ability to build trust and rapport with clients is crucial.
- Ability to travel locally within Queens
Nice To Haves
- Experience in outreach, social services, healthcare navigation, or community engagement (preferred)
- Community Healthcare Worker (CHW) certification strongly preferred
- Bilingual skills a plus (Spanish strongly preferred)
Responsibilities
- Conduct in-person outreach in laundromats (primarily) and other community settings
- Engage residents in conversations about benefits/entitlements such as: SNAP, Medicaid, and public assistance
- Provide education on available healthcare and community-based supports
- Assist with referrals and linkages to care coordination
- Assist with referrals to primary care and outpatient behavioral health
- Build trust with community members through respectful, culturally responsive engagement
- Track outreach efforts and maintain accurate documentation
