As a Community Health Worker, you will serve as a liaison between health and social services and the community, identifying health-related issues, collecting data, and discussing concerns. The region of service will be from downtown Atlanta to Decatur, GA. This role is part of CenterWell Senior Primary Care, which provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment, and more.
