Greater Portland Health is seeking a dynamic individual who will join us as a Community Health Worker. The successful candidate will have a gravitational pull towards helping others. This individual will facilitate access and improve the quality of health care for our patients through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. Greater Portland Health is mission-driven to provide health care services for everyone in our communities.

Requirements

Level 3 English required.
High school diploma or GED equivalent required.
Knowledge of Microsoft Word and Excel required.

Nice To Haves

Experience with electronic health records desired.
