The Community Health Worker is a liaison with resources in the communities. They play a vital role in improving community health by providing a connection between health providers and community resources as well as educating patients on how to reduce health risk factors. FOR FQHC (FOSTER CARE) The Community Health Worker - Foster Care is a liaison with resources in the communities. They play a vital role in improving community health by providing a connection between health providers and community resources as well as educating patients on how to reduce health risk factors. They focus on children in foster care programs, children who were adopted from the foster care program, the children involved with the Division of Youth Services or Juvenile Justice systems, and their families. Essential Job Functions: Facilitate communication and patient empowerment in interactions with healthcare/social service systems. Troubleshoots any barriers the patient might have in adhering to care. Engage in the community through outreach to recruit and engage high-risk patients. Serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease. Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as community education, social support and advocacy. Collaborate with patients and their care teams to create solutions to overcome barriers to achieving healthcare goals. Connect patients with community resources and link patients with health center services as appropriate. Follow-up with patients via phone calls, home visits and visits to other settings where patients can be reached. Performs general support activities and carries out special projects as required. Conduct public education activities to raise awareness of the availability of qualified health plans (QHPs) Distribute fair and impartial information concerning enrollment in QHPs Facilitate enrollment in QHPs by assisting individuals in reviewing their health coverage options as well as complete eligibility and enrollment forms Provide information in a manner that is culturally and linguistically appropriate to the needs of the population being served by the marketplace. Other duties as assigned. FOR FQHC (FOSTER CARE) Essential Job Functions: Assist foster parents in accessing primary medical, dental, and behavioral health care for children in their care with a focus on getting children seen for required initial and follow up visits Conduct SDOH screening utilizing the PRAPARE Assessments to determine SDOH needs of children and foster families annually or more frequently, if change in foster child placement Provide care coordination, which may include but is not limited to facilitating care transitions, supporting the completion of referrals, and providing or confirming appropriate follow-up. Make referrals and connections to community resources to help individuals and families meet basic social needs. Other duties as assigned.
