About The Position

This position will support data transfer and entry activities in EPIC and or the project tracking software as needed and provide high quality health navigation and patient education via telephone for qualified patients who are part of a grant from the Substance Abuse Mental Health Administration (SAMHSA). And other grant-related duties, as assigned. Staff will build and maintain internal and external organizational relationships, document interactions, assist with data collection, identify and share community-based resources, and other grant-related duties, as assigned. Services provided through this project are delivered via phone/telehealth and at in-person group meetings as scheduled. Because this position is funded 100% by the grant, this individual can only perform work related to the grant project. IMPORTANT: This is a time-limited grant-paid position. Funding from the SAMHSA is for the project period: 9/30/24 to 9/29/27. This position cannot be guaranteed after that time.

Requirements

  • Experience in community-based settings as community health worker or similar position, and data entry/customer service.
  • New graduates of the Community Health Worker Training Program in Kettering College may be accepted, with a recommendation from their instructor.
  • The ability to interact with and educate people from various backgrounds.
  • Strong verbal communication and active listening skills.
  • Detail oriented, especially with patient documentation and tracking.
  • The ability to remain humble and deliver programming in a culturally appropriate way. Must value diversity.
  • Must be willing to learn how to use the electronic health record (Epic) and keep excellent documentation in that system. Epic experience preferred.
  • Proficiency in Microsoft Office applications required.
  • Must collaborate well with the project and grant management teams.
  • Highly motivated and can work independently.

Nice To Haves

  • Certification preferred but not required.
  • Epic experience preferred.

Responsibilities

  • Support data entry into Epic and grant tracking application/software. Data should remain accurate and error free.
  • Coordinate meetings and engagement with community partners
  • Via telephone contact primarily from office, provide high quality health navigation, patient education and health coaching for qualified patients who are part of the grant-funded project by assisting them with access to medical services, health insurance, substance use disorder treatment and other resources and services to improve health outcomes; support health goal selection and provide education about pain management. This is a telephonic position, with care provided via telephone primarily in the office setting. Engage in group facilitation and patient education for in-person group meetings as scheduled.
  • Ensure that any data needed for grant reporting is collected accurately and given to the Project Manager on time.
  • Following processes and standards established for delivering telephonic care for patients served by the grant.
  • Ensure that all interactions are documented appropriately.
  • Attend appropriate internal meetings.
  • Ability to deliver services in a patient-centered, culturally competent manner.
  • Stay informed about current trends and developments around SUD, substance use and pain management.
  • Collaborate with the Project Manager for purchasing, etc.
  • Collaborate with Project Manager and grants management team to assure activities comply with federal grant rules.
  • Maintain timely Time and Effort reporting.
  • Some local travel may be required. Travel to attend an annual in state conference is encouraged
  • Other grant-related duties as assigned.
