About The Position

Our Community Health Worker helps increase access to health services for people through outreach, encouraging engagement, education, and peer support. This role offers competitive wages based on experience and up to 3 weeks of PTO in the first year, with the added perk of no nights or weekend work.

Requirements

Completion of Community Health Worker (CHW) Training required
Seasonal Flu Vaccine
Experience working with diverse populations and adapting communication styles as needed

Responsibilities

Conduct patient and family assessments to identify needs such as food, housing, transportation, financial stability, and access to healthcare
Identify gaps in care, including missed primary care visits, preventative care needs, and inappropriate ER use
Document assessments and interactions in the patient record
Coordinate outreach efforts using phone calls, text messaging, mailings, home visits, and community events
Encourage and support patients in managing chronic conditions and engaging in preventative care
Collaborate closely with primary care providers, care managers, and support staff to improve patient outcomes
Address barriers to care and assist patients in accessing timely services
Educate patients on available resources, including financial assistance, insurance programs, and sliding fee discounts
Connect patients with community services and support programs
Build and maintain partnerships with community organizations
