The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally-appropriate care and services. Requirements
- High school diploma or GED preferred.
- If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required.
- Must be a resident of the community being served with good knowledge of the resources of their community.
Nice To Haves
- Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
- Basic computer skills preferred.
