About The Position

Community Health Workers (CHWs), who work within the community and in health care settings, empower patients and their caregivers to improve health outcomes by providing culturally sensitive education, peer-based support, and connections to primary care and community-based social supports. The OB Community Health Worker will receive referrals from medical providers to support high-risk pregnant clients or unstable medical or psychosocial status to connect to clinical and community services to support their health and well-being and to reduce stress. Participating families receive comprehensive information from their CHW regarding: 1) understanding their health conditions, 2) knowing how to access health care for them and their child, and 3) learning about self-management to keep their and their child's condition under control. CHWs also help families connect to services for issues such as food insecurity, immigration, housing, education, and employment. This role will work specifically with pregnant and parenting participants in the NYP Queens health system.

Requirements

  • Bilingual (English/Spanish).
  • 2+ years CHW experience or comparable experience.

Responsibilities

  • Manages designated caseloads and meets all stated program goals.
  • Conducts home visits to provide in-person support and referrals to services.
  • Will be in the hospital on a weekly basis to meet with patients, providers, and support the program.
  • Will chart in the electronic medical record system (EPIC) all necessary information based on client work.
  • Offers practice-based support and information sessions at assigned patient-centered medical home (NY Presbyterian Queens), Public Health Solutions.
  • Collaborates with healthcare team throughout the patient-centered medical home and hospital.
  • Completes appropriate documentation of assessments and educational sessions at program intervals.
  • Maintains organized filing system and submits all paperwork and documentation to program staff by designated deadline.
  • Facilitates/hosts at least 2 community-based workshops per year.
  • Conducts community-wide outreach and assists with event planning, including workshops and health fairs.
  • Attends all required staff meetings and training.
