About The Position

Ohana Pacific Health is Hawaii's largest, locally owned post-acute healthcare company, dedicated to transforming healthcare throughout Hawaii with its "Ohana Experience" culture of excellence and genuine care. Kalele Care Services, founded in 2020 by Ohana Pacific Health, addresses the complex healthcare needs of the community, with a particular focus on assisting Kupuna (elderly) to live longer and healthier independent lives. The organization is seeking a caring and passionate Community Health Worker to support patients in navigating and accessing health and community services. This role emphasizes providing culturally relevant care, support, and respect for kūpuna, with services delivered primarily in-person in hospital and short-term rehabilitation settings, and in the community both in-person and telephonically.

Requirements

  • High school diploma or equivalent.
  • Proficiency in basic computer skills including but not limited to, navigating web browsers, electronic medical record systems, computer systems and applications, Microsoft office suite.

Nice To Haves

  • Associate or Bachelor’s degree in social work or other healthcare related field, community health worker certificate, or nurse aide training.
  • Experience in healthcare or home health field or work with the geriatric population.

Responsibilities

  • Support in carrying out treatment goals or care plans created by advanced practice provider (APP).
  • Assists the patient in improving healthcare access and promotes patient knowledge of health and behavior change.
  • Supports patients with community referrals and arranges for follow-up services.
  • Collaborates with APP in developing patient treatment goals related to personal wellness, access to care, engagement with health resources and services, and self-management of conditions.
  • Responds to patient inquiries and assists patients with the completion of paperwork.
  • Facilitates remote visits between patient and provider using telephone or video conferencing.
  • Educates patients on care processes and provides counsel to reduce anxiety and fear.
  • Identifies patient barriers to care such as financial barriers, transportation, insurance, and language barriers, and refers patients, as needed, to additional resources to support engagement in care.
