About The Position

This position will conduct outreach and education services specifically for individuals who are at high risk for cancer and works with a team of health care professionals to provide clients with opportunities to manage and improve their health by facilitating care coordination through cancer screenings and treatment, enhancing access to community-based services, providing cancer education, and addressing social determinants of health. This individual will work with individuals as part of an integrated team of health care providers. Significant travel is required. Up to 50% of the time, this team member will be in the field making home visits and working with community-based agencies. Work is performed in the office area and remotely in other locations to provide flexibility for meeting with clients, community partners, vendors, etc. within Cotton, Jefferson, and Stephens’s counties in Southwest Oklahoma. This position is grant-funded.

Requirements

  • Communication skills including fluency in oral and written English.
  • Basic computer skills including the ability to send/receive/email, navigate information technology associated with the position, and use Electronic Health Record information tools.
  • Excellent written, verbal, and interpersonal communication skills
  • Ability to adapt procedures, processes, tools, equipment, and techniques to accomplish the requirements of the position.
  • Advanced, fingerprint background checks are required for this position.
  • Works effectively in team environment, participating and assisting their peers.
  • Ability to organize, coordinate, and demonstrate judgement, professionalism, diplomacy, assertiveness, and initiative.
  • Frequent departmental interfaces with internal and external customers to coordinate and provide optimal level of program services.
  • Knowledge and ability to develop and implement policies and procedures.
  • This position requires effective time management skills, ability to comply and operate independently within organizational policies and/or direction of supervisor.
  • High school diploma or equivalent.
  • At least two years of work experience in healthcare.
  • Excellent skills in using Microsoft Word and Excel and Google Documents.
  • A current, valid driver’s license and automobile liability insurance must be maintained.
  • Must have reliable transportation.
  • This position requires respirator and fit testing.
  • As a condition of employment, vaccinations are required per DRH Policy. Medical and Religious Exemptions are available upon request.

Responsibilities

  • Develops and maintains collaborative partnerships with health systems, community clinics, healthcare providers and other community partners.
  • Supports patient engagement and recruitment efforts for cancer screenings.
  • Interacts and engages with the community and oncology patients to promote the prevention and early detection of cancer.
  • Maintains accurate county and community data for service area, diligently recording all encounters in grant database.
  • Collaborates with Cancer Care Coordination Network team to report all outreach encounters and program-related engagements as required by grantors.
  • Conducts, attends, and represents the Cancer Care Coordination Network at health fairs and educational events in Stephens, Jefferson, and Cotton counties, delivering informative and educational programs for patients, family members, and/or the general public.
  • Works with Cancer Care Coordination Network on marketing efforts which includes outreach to clinics having possible cancer candidates and arranging presentations to these or other potential community partners as needed.
  • Assists community members with barriers to care coordination access; assists with connecting members to community partners as needed.
  • Attends meetings with Cancer Care Coordination Network to address goals, barriers to services, and program improvement, assists with weekly, monthly, and annual reporting.
  • Track and report client encounters and maintain required records as directed.
  • Maintain appropriate confidentiality of information.
  • Act as a patient advocate.
  • Provide client follow-up and monitoring, including phone calls and home visits with patients as needed.
  • Assist patients in understanding their insurance and medication coverage.
  • Assist patients in applying for medication assistance programs as needed.
  • Demonstrates knowledge and support of DRH Health's mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
  • Actively participate in required training and professional development activities.
  • Maintains professional and technical knowledge through continuing education opportunities including internal and external educational offerings.
  • Regular attendance and punctuality for scheduled shifts.
  • Utilization of assistive devices for lifting is mandatory.
  • Must adhere to safety protocols at all times.
  • Per DRH policy, all required conditions of employment must be met and maintained including required vaccinations.
  • Implement DRH Standards of Behavior and exhibit behaviors consistent with DRH core values.
  • Performs other related duties as assigned.
