About The Position

Sentara Martha Jefferson Hospital is hiring a Community Health Worker for a Full Time, Day shift position in Charlottesville, VA! This is a Monday to Friday position- hours are 8am to 430pm. No weekend requirement. The Community Health Worker is in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management. The ICM team at Sentara Martha Jefferson has 13 members including MSW, Community Health Workers and RN Case management staff. This is a collaborative environment with a patient- first mind set. Requirements

  • Valid Drivers License
  • High School Diploma or Equivalent

Nice To Haves

  • Bilingual in English and Spanish is preferred.
  • Experience working in a community-based setting preferred.
  • Knowledge of medical terminology preferred.
