The Community Health Worker (CHW) at Prisma is a trusted advocate, educator, and navigator, playing a pivotal role in strengthening Prisma’s status-neutral HIV prevention and care continuum. This grant-funded position focuses on meeting individuals where they are and connecting them to affirming services. Key responsibilities include leading linkage-to-care workflows, implementing education initiatives, and managing grant-required reporting and data tracking. The role involves robust community outreach and relationship-building, requiring flexibility to support evening and weekend events. The CHW will coordinate with Prisma’s clinical/testing staff and program partners to facilitate testing events through education, navigation, and warm handoffs, though they will not administer HIV testing or supervise staff. This position is integral to the Prisma Community Care mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities, particularly people of color, 2SLGBTQIA+ and Queer individuals, and those affected by HIV.
