Community Health Worker
About The Position
Community Health Worker will work primarily with specific target populations including but not limited to non-English speaking populations of geographic target areas. CHW will work closely with internal and external stakeholders or partners including but not limited to clinical/medical providers, primary care teams, and other agencies to improve patient care and outcomes. Patient outreach interactions may be telephonic, video/digital, or in-person generally at an office site. Depending on department location CHWs may need to be bilingual or be knowledgeable of specific department terminology such as medical or dental.
Requirements
- Completes and maintains all competencies and trainings as required for role.
- High School Diploma - OR - GED - OR - Relevant experience may be considered in lieu of education
