About The Position

On Belay is seeking a motivated, passionate Community Health Worker to assist our provider partners’ Medicare patients. The goal of the Community Health Worker is to assist identified patients with developing their health goals and creating a plan to manage real life issues that keep them from achieving those goals. The Community Health Worker will collaborate with other team members to conduct needs assessments and identify and respond to barriers to their health and wellness. We are looking for a Community Health Worker to join our care management team in the South Shore/Greater Attleboro, Massachusetts area. The position is a full-time position (40 hours), Monday-Friday daytime hours. The position is hybrid with the expectation for half-time in the office.

Requirements

  • Minimum of one year of relevant work experience in community health
  • Deliver sensitive and responsive services to patients from diverse cultures and socioeconomic communities.
  • Assist patients in finding community resources related to Social Determinants of Health (SDOH) needs, including housing, food, transportation, utilities, medication costs, adult daycare, etc.
  • Work in partnership with patient and caregivers to identify and address barriers to care and help develop skills to achieve better health outcomes
  • Previous community health experience working with patients with complex medical, social, and psychiatric/behavioral needs
  • Must be a self-directed individual able to work independently without direction and able to prioritize workload
  • Basic computer skills including working knowledge of Microsoft Office products
  • Experience working in EMR’s such as EPIC and Athena

Nice To Haves

  • Associate's degree in human services, Public Health, or similar degree
  • Massachusetts Board Certification in CHW (or obtain within 6 months of hire)
  • Knowledge of health plans, Medicare, and Medicaid
  • Bilingual (English/Spanish)

Responsibilities

  • Develop trusting relationships with patients by providing support and advocacy to help achieve health goals
  • Assist patients in accessing community resources through referrals to appropriate agencies (e.g., food stamps, meals on wheels, subsidized housing, free legal services, Medicaid/Medicare applications, support groups, etc.)
  • Meet with patients in patient-centered and patient-preferred locations (e.g., center, facility, home, community setting).
  • Manage referrals as defined by the care management program and collaborate with social work or care manager on action plan
  • Engage with high-risk individuals
  • Assist completion of applications to access eligible benefits
  • Assist patients in scheduling medical appointments as needed
  • Perform wellness checks in homes
  • Participate in communication with care team (e.g., family members, care givers, medical providers, community-based organizations) as deemed appropriate
  • Document interactions with patients in medical record and care management platform
  • Any other related duties assigned at manager’s discretion
