About The Position

The Community Health Worker (CHW) will oversee and support the JeffMD Clinical Experience students as they learn to speak with and advocate for their patients. The CHW will coach and guide these first-year medical students through their experience working with patients on social needs related to their health. The CHW will also lead discussions with students during case-based learning sessions pertaining to social needs. When the students are not in clinical sessions, the CHW will continue to interface with patients and their families in order to facilitate access to community resources and services. The CHW will provide social support, informal counseling, education on the various community resources and services, and advocate for individual and community health needs. The CHW will be a liaison between the patient/families and the community service agencies. The CHW will coordinate with service providers and health plans, as appropriate, to obtain care for the patient. The CHW will provide support to the selected practice care coordinators and providers through a team-based care model. Requirements

  • High School Diploma or GED
  • Prior experience in community outreach preferred.
