About The Position

Queens Healthy Start is seeking a bilingual Community Health Worker who will provide home visiting and case management services to low-income parents and their infants under 18 months of age residing in South Queens, including the neighborhoods of Jamaica, Howard Beach, Queens Village, and Far Rockaway.

Requirements

  • Associate’s degree preferred, minimum requirement high school diploma/GED.
  • Bi-lingual preferred (Spanish and/or Haitian Creole)
  • Experience working with families including migrants who reside in the shelter system
  • Experience in home visiting preferred
  • Proficient in Microsoft Office Suite (Outlook, Teams, Excel, Word) and comfortable learning new software quickly
  • Ability to travel to different neighborhoods within Queens including Far Rockaway.

Responsibilities

  • Provide case management for up to 30 clients at any given time through one-on-one interactions with families including home visits, office visits, or community visits (i.e. meeting at a local library or park)
  • Provide health education and parenting support using the Parents as Teachers curriculum
  • Conduct basic health and social assessments, assist families to identify needs, establish goals, provide basic health information, and make appropriate referrals through weekly, biweekly or monthly home visits
  • Provide advocacy, support and follow-up to determine if services are received and help reduce barriers to receiving support, and assist families with health behavior changes
  • Provide ongoing follow up with clients, utilize basic motivational interviewing and goal setting with patients/families around their health and personal goals
  • Maintain documentation of home visits, client interactions, and assessments using case management software, Well Family System GoBeyond
  • Occasionally conduct or assist in conducting educational support groups and workshops
  • Conduct community outreach (attending baby showers or other tabling events) on behalf of the Queens Healthy Start program as needed
  • Perform other duties assigned.
