About The Position

The Community Health Worker (CHW) serves as a trusted frontline public health professional who connects individuals and families impacted by substance use disorder (SUD) to health, social, and recovery-support services. Working within The HUB Recovery Community Center, the CHW addresses social determinants of health, reduces barriers to care, and supports participant stabilization through education, navigation, and follow-up. This position supports the Floyd County Health Department’s responsibilities under Kentucky Public Health Transformation, emphasizing prevention, health equity, and community-based interventions.

Requirements

  • High school diploma or equivalent required; associate degree or relevant certification preferred
  • CHW certification or willingness to obtain certification
  • Experience working with vulnerable populations, public health, social services, or community outreach preferred
  • Strong communication, organizational, and relationship-building skills

Responsibilities

  • Conduct outreach, intake support, and needs assessments for HUB participants
  • Assist participants in accessing healthcare, behavioral health, SUD treatment, harm reduction services, housing, food assistance, transportation, childcare, and employment resources
  • Provide warm handoffs and follow-up to ensure successful connections to services
  • Deliver culturally responsive education related to overdose prevention, communicable disease prevention, chronic disease management, and wellness
  • Support participants in understanding care plans, appointments, and referrals
  • Promote engagement in HUB programs, recovery supports, and workforce readiness activities
  • Coordinate with peers, case managers, healthcare providers, and community partners
  • Document services, referrals, and outcomes in accordance with health department and grant requirements
  • Support data collection for performance measurement and reporting
  • Participate in multidisciplinary teams addressing public health priorities
  • Assist with community events, outreach initiatives, and emergency response efforts as needed
