About The Position

The Community Health Worker provides culturally responsive, community-based services that address social drivers of health and support individuals in accessing health care, behavioral health services, and essential resources. This role focuses on care coordination, system navigation, health education, and advocacy to improve health outcomes and overall stability in alignment with Medi-Cal CHW benefit requirements.

Requirements

  • High school diploma or equivalent.
  • Minimum two years of experience providing community-based services, care coordination, or related human services work.
  • Strong written and verbal communication skills.
  • Bilingual in English and Spanish.
  • Ability to manage multiple responsibilities and maintain timely documentation.
  • Basic computer proficiency and experience with electronic records.
  • Valid California driver’s license, reliable transportation, and proof of insurance.

Nice To Haves

  • Community Health Worker certification or eligibility for certification.
  • Associate degree or coursework in public health, human services, social work, or a related field.
  • Familiarity with Medi-Cal, managed care environments, and SDOH frameworks.

Responsibilities

  • Participant Engagement and Assessment Conduct intakes, eligibility verification, and informed consent processes.
  • Administer Social Drivers of Health (SDOH) screenings and needs assessments.
  • Collaborate with participants to develop individualized action plans using SMART goals.
  • Engage participants using a strengths-based, trauma-responsive, and culturally relevant approach.
  • Care Coordination and System Navigation Provide health education and information tailored to participant needs.
  • Assist participants in accessing medical care, behavioral health services, benefits, housing resources, employment supports, transportation, and other community services.
  • Coordinate referrals and follow up to ensure services are accessed and effective.
  • Accompany participants to appointments when necessary to support engagement and continuity of care.
  • Advocate on behalf of participants to reduce barriers to care and services.
  • Documentation, Billing, and Compliance Document all CHW services accurately and timely in the electronic health record.
  • Utilize PIP (Problem, Intervention, Plan) format for all progress notes.
  • Ensure documentation meets Medi-Cal, managed care, and internal compliance standards.
  • Track service delivery, referrals, and outcomes as required.
  • Participate in documentation reviews, audits, and quality improvement activities.
  • Collaboration and Program Support Participate in case conferencing, team meetings, supervision, and trainings.
  • Collaborate with peer support staff, providers, and community partners to support coordinated care.
  • Maintain professional boundaries and adhere to organizational policies and procedures.
  • Other duties as assigned.
