About The Position

The Community Health Worker (CHW) is a trusted advocate, educator, and navigator dedicated to strengthening Prisma’s status-neutral HIV prevention and care continuum. This role involves meeting individuals in the community and connecting them to affirming services. It is a grant-funded position that primarily focuses on leading linkage-to-care workflows, implementing education initiatives, and managing grant-required reporting and data tracking. The role requires flexibility to support evening and weekend outreach and community events. The Community Health Worker is integral to the Prisma Community Care mission, which aims to provide affirming and inclusive services to promote well-being and advance health equity for diverse communities and all individuals seeking compassionate care, with a particular focus on people of color, 2SLGBTQIA+ and Queer individuals, and those affected by HIV.
